20% off scaling and root planing
50% off Arestin placement (unlimited)
Family Plans
(For immediate family members — parents & their children up to age 18)
$695 – 2 members
$745 – 3 members
$895 – 4 members
Additional members: $110 per additional member requiring periodontal cleaning & maintenance
Membership program exclusions and limitations
Please note this is a discount plan, not dental insurance. Payment is due at the time of service, and plans are exclusively honored at Lighthouse Dental Studio. A $50 reinstatement fee applies if your plan lapses.
A membership plan cannot be used:
In conjunction with another dental plan, dental care credit or financing program such as CareCredit
For treatment which, in the sole opinion of our doctors, lies outside the scope of his capability
For referrals to specialists
For costs of dental care which are covered under automobile or medical insurance
For services of injuries covered under workers compensation
